State of Iowa Mandatory requirements:
Kindergarten: All students entering kindergarten fall 2019 must have on file by the first day of school a Certificate of Immunization with proof of the required 4-6 year old vaccines, Iowa Certificate of Dental Screening signed by a dental professional, (Rikki Hetzler the I-Smile coordinator will be at our Kindergarten Round-Up in April and will be completing these brief exams) Certificate of Vision Screening signed by an eye care professional and a reported lead level( done usually at the 12 month or 2 yr well check). If an up to date vaccine record is not on file by the first day of school your student will be excluded until proof is submitted.
3rd grade: All students entering 3rd-grade fall 2019 must submit a Certificate of Vision Screening signed by an eye care professional. Letters informing parents of this as well as a Certificate of Vision Screening were mailed out at the end of February.
Current 6th and 11th-grade students: Louisa County Public Health will be conducting a vaccine clinic at the elementary and high school for students entering 7th and 12th grades in the 2019-2020 school year. They will be providing the Iowa mandatory Tdap and Meningococcal (MCV4O/MCV4P) vaccines. Letters regarding this mandate as well as the LCPH authorization forms were mailed out March 1, 2019, to all parents of students that need these completed. I recommend taking advantage of this clinic as it is little or no cost to you depending on insurance. The form needs to be turned into me no later than March 29th, 2019. The date for the clinic to be determined. A record of these vaccines must be on file by the first day of school in August 2019. If not on file your student will be excluded from school until proof is submitted.
9th grade: All students entering 9th-grade fall 2019 must submit a Certificate of Dental Screening signed by a dental professional. Letters informing parents of this were mailed out at the end of February.
